Sometimes if you refresh the page they are in phase, sometimes not. They remain perfectly in sync - if they load a few frames out of phase (sometimes it's OK), they stay that way.
The odd thing is that these two are always in phase for me, but the mirrored one is saved directly in the hoard (it's the image from the poll):

Perhaps it's a simple matter of making your code run in zero milliseconds.

I did think about implementing a cache - I think the best way to do it would be to add an extra table to the database to remember which smilies have been cached and a separate folder on the server to store them.
It's not just a matter of the time the script takes to run - there's also the issue (when your post calls for an original+mirror pair of smilies) of the same (gif) file being accessed twice at the same time: directly by Apache and indirectly via the script.
For some reason, the smiliepaths on are stored with the full http://www.... etc. path whereas on other vBulletin implementations I've looked at, they are just stored as /images/smilies/... I don't know if this means the server takes longer to fetch them.
What happens if we put {mirror=0} {/mirror} around the original smiley

On a related note it's kind of a shame that the mirror tag doesn't work with html-cited images or IMG code. I had to cite the first batch from the script on ceptimus' server. Ah well.

But it does!
You just paste the URL that you would normally paste inside {img} tags into the {mirror} tag. If you want to line up an image with its mirror, then it's better to use {mirror=0} rather than {img} for the original, as {mirror} uses the same vertical alignment as smilies, which is different to the one used by images.
It does only work (at the moment) for .gif files though. See below for example.
